The U.S. Food and Drug Administration (FDA) continually monitors the safety of drugs and medical devices even after they have been approved and hit the market. This post-market surveillance is crucial for ensuring the ongoing safety and efficacy of these products. To enhance this surveillance, researchers, including members from the FDA’s Center for Drug Evaluation and Research, are proposing the use of artificial intelligence (AI), specifically large language models (LLMs), to improve the detection of potential safety issues.

The suggestion was detailed in an analysis published in JAMA Network Open, highlighting how LLMs could potentially be integrated into the FDA’s existing surveillance system, known as Sentinel. Sentinel primarily uses clinical records and insurance claims to monitor the safety of regulated products. The system helps the FDA make informed decisions about necessary adjustments to drug labels, the formation of advisory committees, and the dissemination of drug safety communications.

Large language models could revolutionize Sentinel by enabling it to analyze a broader array of data sources, including electronic health records (EHRs) which contain voluminous amounts of unstructured text data. Currently, Sentinel’s capability is somewhat limited to structured data inputs, which constitute a fraction of the potential data available on drug performance and safety. By harnessing AI, the FDA could tap into expansive pools of textual data from EHRs that were previously challenging to analyze on a large scale. Additionally, AI could potentially extend surveillance capabilities to include information from social media posts and clinical databases that reference the use of certain drugs, providing a more comprehensive view of a drug’s safety profile across different platforms and user experiences.

However, the integration of AI and LLMs in drug surveillance does not come without risks. One significant issue highlighted by the researchers is the prospect of AI-generated ‘hallucinations.’ These hallucinations refer to instances where LLMs might produce false information, which could misrepresent the safety risks associated with a drug. If the model inaccurately overstates or understates the risks, it could lead to poorly informed decisions about drug safety, potentially endangering public health or causing unwarranted panic.

Hallucinations in LLMs occur because these models, which generate coherent and contextually appropriate text based on the data they have been trained on, can sometimes make errors in generating accurate content. The model’s outputs depend heavily on the quality and range of its training datasets, and if these datasets have gaps or biases, the model’s outputs might be incorrect or misleading.

Therefore, while the incorporation of AI into the FDA’s drug surveillance system could considerably increase the breadth and depth of safety monitoring, it is essential to approach this integration judiciously. Critical to the success of such a venture would be rigorous validation phases where the outputs of AI are continuously checked for accuracy. Moreover, developing robust protocols to quickly identify and rectify any false information AI might generate will be crucial in mitigating risks. Balancing these risks against the potential benefits will require careful planning, continuous oversight, and a deep understanding of both the capabilities and the limitations of AI technologies.

Given the rapid evolution of AI capabilities, the FDA’s move towards integrating advanced AI tools like LLMs into its surveillance practices could set a precedent in regulatory science, making drug safety monitoring more dynamic and comprehensive. This could ultimately lead to improved health outcomes by enabling quicker and more precise identification of potential drug-related risks, thus allowing for timely regulatory actions. However, the journey towards the full realization of AI’s potential in regulatory science must be navigated with a clear focus on maintaining the accuracy and reliability of data guiding public health decisions.
